A community paramedic program is helping patients see more success in managing their healthcare, by tending to that healthcare in their own homes. Waupaca’s ThedaCare Physicians and Gold Cross Ambulance teamed up to create the program, after seeing it used successfully in the Fox Valley. The program works like this….. A patient’s doctor submits a request to the Community Paramedic program team, which determines if the patient is a likely candidate to benefit from it. Next, a paramedic with the program sets up a home visit, then after the visit, lets the primary care physician know what happened during the visit, and if any changes to the patient’s care plan were made or recommended. The program targets patients who have had three or more emergency room visits in the last 6 months, or six or more in the last year. The goals of the program include decreasing the number of emergency room visit, helping patients better manage their health care, and reduce the stress of doctor’s appointments by having paramedics come to patients’ homes.
